When the Additive Assistant Calculator results are accepted and stored into the TPN order,
any additives that were originally in the TPN order will retain the original order units.
For example, if the TPN order contained an additive that was ordered in mEq/Kg, the appropriate dose in mEq/Kg will be used.
This functionality overrides the Additive Assistant Calculator Desired Electrolyte Totals units and the Additive Order Units selection settings.
Additives that were not already in the original TPN order and are being added to the order by the Additive Assistant Calculator are handled as follows:
- If all the ions associated with a particular additive are ordered with the same per units,
the additive is added to the TPN order using the default additive solution dose unit and same per unit.
- If the ions associated with a particular additive have different per units,
the additive is added to the TPN order using the default dose unit and a blank (per container) per unit.
- If the Additive Order Units "mLs" option is selected, the additive is added to the TPN order using mLs per container units.
Create a new TPN order and enter the Additive Assistant Calculator.
Order 5 mEq/Kg of K+ and Ca++ as shown here:
Let's say that the results when Calculate is pressed are Calcium Gluconate and Potassium Acetate.
When the "Accept this Proposed Order" button is pressed, the Calcium Gluconate will be entered into the TPN order in mEq/Kg units,
but the Potassium Acetate will be entered in mEq (per container) units.
This is because the AC- units in the Additive Assistant Calculator were per container.
In order to make both of the additives entered into the TPN order be in /Kg units, the Additive Assistant Calculator settings would need to be as shown here:
Now all the ions in the additives are in /Kg units, and as a result they will be added to the TPN order in mEq/Kg units.
